Filling out the Program of Study electronic form for a Ph.D. Student

The program of study form can be found on the Graduate School website Forms (k-state.edu).

Here are the questions on the electronic form and how you will answer them. On the KSU Credits, please list all of the PhD courses you have taken at K-State and the semester you took them and then list the ones you will be taking and what semester you plan to take them. Please list the KSU Credits in the same order as they are listed on the Excel Course Worksheet file to make it easier for us to check.

All of the research credits should be listed in one entry as shown in the example below. If you took research credits in different semesters enter “Multiple” as the semester taken.

If you have been awarded a master’s degree previously (as is true with nearly all students), please list your degree under the transfer of credits. Do NOT list specific courses that you took during your master’s degree. Instead, follow the instructions on the grad school form to list the course number as “degree”, the course title as “name of degree awarded” (ex. master of science) and for “semester taken”, please list the semester the degree was awarded. If you are using specific courses from your master’s degree to count for your PhD requirements, please indicate this on your Excel Course Worksheet. You do not need to enter this information on the grad school POS form though. Steph will add a comment to indicate that we are using courses from your master’s degree to count for specific PhD requirements based on the Course Worksheet that Dr. Hendricks approved.

Once you submit this form it will be routed to Steph first to review, then all of your committee members and Dr. Hendricks. You can see the status of the form by looking at Etrieve → Activity and selecting the form and clicking History.
